Dive Team Registration Form - 2026
Directions: This form must be completed in full by all parents/guardians of current and prospective dive team members before your child may join the dive team and/or participate in any practices or meets.

All registered divers will get a DH Dive t-shirt and DH Dive magnet.
The preferred method of payment is credit card, which you may enter as you complete your registration. If you prefer you may pay by check made payable to “DHARA Summer Swim and Dive Program”. Please get your checks to Kirsten Robbins no later than May 18, 2026. Your registration is not complete until payment is received.
Costs:
The cost is $130 per diver and there is no longer sibling discount.
